First Visit

Initial office visits are designed to suit your dental needs. When requesting an appointment please specify your preference for a traditional cleaning and exam, consultation or dental emergency. Upon scheduling your first dental appointment, a selected time will be reserved for you. A twenty-four hour notice of cancellation is respectfully requested.

On arrival, the required medical, dental and HIPPA forms will be provided for your completion. Please be prepared to provide any insurance information that you would like us to submit on your behalf. Our well-qualified staff has the knowledge and experience to determine the best treatment for your personal dental needs. We look forward to meeting you!

PLEASE TO BE PREPARED TO PROVIDE:

  • A list of medications you are presently taking.
  • If you have dental insurance, bring the necessary information. This will save time and allow us to help you process any claims.
  • Recent x-rays from previous dentist (within the last two-three years).
  • A parent or guardian must accompany all patients under the age of 18.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e. antibiotics, for pre-med.)
