First Visit

As a new patient your initial visit will include a comprehensive exam, and any necessary x-rays. During the exam Dr. Wang will discuss your medical and dental history and address any concerns you may have. When requesting an appointment please specify your preference for a routine preventative visit, consultation or dental emergency. Upon scheduling your first dental appointment, a selected time will be reserved for you. Any appointment cancelled with less than a twenty-four hour notice is subject to a late cancellation fee.

Upon arrival, the required medical, dental, HIPAA and financial forms will be provided for your completion. Our well-qualified staff has the knowledge and experience to determine the best treatment for your personal dental needs. We look forward to meeting you!

PLEASE TO BE PREPARED TO PROVIDE:

• A list of all medications you are presently taking.
• If you have dental insurance, bring the necessary information. This will save time and allow us to help you process any claims.
• Recent x-rays from previous dentist (within the last two-three years).
• A parent or guardian must accompany all patients under the age of 18.

Please alert the office if you have a medical condition that may be of concern prior to appointment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e. antibiotics for pre-med.)
